AI in medical imaging is moving beyond simply assisting clinicians. Berlin-based Vara has received a landmark European CE certification for an AI-enabled breast cancer screening system that can autonomously report mammograms it determines to be normal – without radiologist review.
This represents an important shift from AI-supported diagnosis toward selective AI autonomy in routine screening.
What makes the approval significant?
• Mammograms classified as normal by Vara’s AI can be reported autonomously, while all other examinations continue to require review by at least one radiologist.
• The system was authorized as a Class IIb medical device under the EU Medical Device Regulation.
• Vara developed ATMON (Autonomous-Triage Monitoring) to continuously supervise AI performance, helping address human-oversight requirements for high-risk AI systems.
• ATMON monitors mammography hardware changes, system health and daily AI performance. If predefined thresholds are breached, the site can automatically revert to full radiologist reading.
The scale opportunity is substantial.
European breast cancer screening traditionally relies on two radiologists interpreting each mammogram. Against the backdrop of radiologist shortages and rising imaging volumes, safely removing normal examinations from radiologist workloads could materially reshape screening economics and capacity.
Vara already has significant real-world scale: more than half of Germany’s organized breast screening program operates on its platform, processing over 250,000 screenings per month.
Clinical evidence is also building. A large study published in Nature Medicine, involving 460,000+ women, found higher breast cancer detection rates with AI-supported double reading compared with conventional double reading.
The bigger story is not simply autonomous AI. It is the emergence of a safety and monitoring infrastructure capable of governing autonomous clinical AI in real-world environments.
That could become a critical blueprint for the next generation of AI-enabled diagnostics.
